Part One: Please write a Social Work Assessment.  In the case analysis, using the template provided you will need to:
 

Introduction: Social context – locate the client within a network of personal, family and community relationships.
Impact of the illness on the person’s life and the lives of significant others.
Describe the person’s current level of functioning – how well are they managing in their current context?
Stressors and strengths in the person’s current situation.
Summary of the situation. What are the key aspects of the case that will shape your work with the client?
Plan for the activity. A short plan of how you would want to work with your client.

 

Part Two: Connecting the case to broader social work and mental health theory and personal values. Outline what theories have underpinned your assessment and intervention plan, and provide a reflective account of your own reactions, including an exploration of personal values and issues to the case presented.
 

 

Why are you meeting Maia?

 

You are a new social worker to the community mental health team and you are meeting Maia, who shares her story with you (Video below). 

 

Maia a 28-year-old mother of a 1-year old son is living with her very supportive family. She has a long history of substance use, including heroin, ice, meth and prescription drugs. She has been to detox and rehab many times.  She was heavily using ice until she found out she was pregnant at 12 weeks – and since then has shifted her life 180 degrees. She is on methadone, has mental health and child protection support, lives with her mother and stepfather and is also supported by the family of her son’s child. She has minimal contact with the father who is still using – one of the reasons she was referred to child protection is because of his behaviour to her while she was pregnant, and she refused to inject.

Maia is keen on working with you on her goals. Maia currently gets a single parent payment from Centrelink, but has no other income. She has had irregular casual roles in retail, but is not working. Her family are her main supports.

Part A – The Assessment Report 

 

Introduction

Person’s name, age, and other important, relevant information.

Client, Maia, 28years old mother to a one-year-old boy, Tyler. Darren is Tyler’s dad, but not been supportive since Maia was pregnant especially on the use of drugs. Maia suffered from depression when her brother Stephen committed suicide a while ago and this resorted in her using heroin, ice, meth, and prescription drugs, Maia was in the university when she started using drug. She got addicted with drugs and has been to several rehabilitation program in the bid to stop drug usage but none of this worked. Maia was heaving on drugs when she realized she was pregnant, and she made up her mind to stop using drug as soon as she knows it’s a boy. Maia would like Darren to be part of Tyler’s life if only he would stop using drugs so they could both support Tyler. Maia lives with her Mum and stepfather who has been of great support to her and her son, also, Darren’s family member has been supportive with Tyler’s care. Maia is trying so well to be the best Mum for Tyler.

 

 

 

Presenting problem

Locating the person in a social context – social networks and family.

 Describe the network of significant personal and family relationships. You might draw a genogram here.

 

 

 

Relevant personal, family, medical, employment and education histories.

This section needs to be very tightly written – clear, concise with lots of concise information. This is where you present a lot of information from which you will later draw conclusions. Make sure this isn’t just your opinions or conclusions.

Personal history

 

 

 

Identifying the person’s understanding of their situation.

 

You can use their words here in this section, as well as give your opinion about how the person understands their situation.  Consider insight, sense of agency, guilt and responsibility.

 

Current level of functioning

 

Relevant concepts here are the coping capacities of the ego – perceptive, integrative and executive functions, role capacity, problem solving capacity, stress management and impulse control.

 

 

Impact of the illness on functioning.

In what ways does the mental illness or disorder impact on the person’s personal and social functioning? Consider significant psychiatric symptoms like depression, anxiety etc.

 

Identifying strengths and stressors

 Consider personal strengths, relationships, family and community resources, evidence of previous coping capacity.

 

Identifying the person’s expectations and goals

 What do they want to happen?

 

 

Reaching a conclusion about what the problem is.

This summary statement is the heart of the assessment process. This section needs to be clear, tight and clearly connected to the information you present in the earlier sections of the report.

 

 

Plans for the future.

 What is proposed as an intervention (if any) for this person. What are you going to do?

 

 

Part B – Reflective Practice
