A ‘Care Plan’ is a comprehensive document that provides students with the opportunity to demonstrate course learning. ‘Care Plan” will include recommendations for community resources and family involvement. Recommendations will promote Quality of Life, dignity, self-determination, and happiness, while also tracking progress and accountability. choose any one Case Studies:
Case Scenario #1: Coen’s Story Coen is a 14-year-old boy with autism spectrum disorder. He lives with his mother in Toronto, Ontario. Coen and his mother moved to Toronto one year ago from a First Nation reserve in Northern Ontario. His parents are currently going through a divorce, and Coen currently does not have any contact with this father. His family is English speaking.
Coen is in grade nine at his local public high school. His current Individual Education Plan (IEP) is primarily focused on academic goals. He attends school most days but will often complain about feeling sick, hoping he will be allowed to go home. He is described by his teachers as impulsive and always on the go, which makes it hard for him to stay focused and in his seat. Despite some of these challenges, he does well in mathematics, and is enjoying learning to cook in his hospitality class.
Coen has a moderate developmental delay. His mother has expressed difficulty accepting his diagnosis and feels overtime he will “outgrow it” and go on to university. His mother is concerned about his lack of peer relationships and would like for Coen to make friends, gain confidence and learn how to socialize with his peers.
Case Scenario #2:
Isaac’s Story Isaac is a 21-year-old man with a developmental and intellectual disability. He currently lives at home with his parents and one younger brother in the southeaster region of Ontario.
Isaac is enrolled at his local public-school and attends school regularly, but this is his last year as he is 21 years old and will no longer be eligible to attend school. His current Independent Education Plan (IEP) is primarily focused on academic skills; however it is outdated and has not been updated since he was in grade 8. Isaac and his family have not participated in an integrated transition planning process or have a plan to support his transition to adulthood.
Isaac’s parents are very supportive and caring and have expressed that they just want their son to be happy. Since they were older when they had him, they are concerned that they may not be able to care for him much longer. His brother is also preparing to move to Montreal to attend University next year. Isaac’s parents are worried about his lack of independent living skills. They are also worried about what he is going to do once at the end of the school year.
Case Scenario #3:
Genevieve’s Story Genevieve is a 29-year-old woman diagnosed with a mild intellectual disability (MID) who has a history of addiction. She was sober for four years from ages 23-28 years. Following the death of her mother as well as losing her job about one year ago she began drinking alcohol heavily. She is currently unemployed and not taking any steps to find a new job.
Genevieve lives in the southwestern region of Ontario and has a large extended family, who live close by but have very little contact with her. Her family has reported frustration with Genevieve often asking them for money and worry about the amount of time she spends going out with friends.
Most of Genevieve’s friends are heavy drinkers or substance users, and her social life revolves around drinking. Genevieve is the single mother to a three-year-old son and wants to get “clean” as she is afraid, she will lose custody of her child. Children’s Aid Society is involved with Genevieve and her son as Genevieve has a criminal record for stealing make-up from the local drugstore.
Assignment Components: This individual Care Plan submission will include a minimum of 5 resource (in the same geographical location as the client) recommendations across broad areas (health, wellness, skill acquisition, social etc.) A thorough description of the resource (contact information, description of services, etc.).

Identifies resources across domains of need (i.e., health, wellness, skill acquisition)

Demonstrates an understanding of the resources identified
Provides a description on how to track client progress with each service
Rationale:
Provides a thorough and clear rationale for supporting each resource recommendation
Demonstrates an understanding of how resources will meet the client needs
Clearly relates each rationale with course theory
Demonstrates a deep understanding of the course theory in rationale
