She presented to your office after being referred by her physician. You just completed an intake interview with her.While talking with her, you learned that Jennise was a rather fearful child. She frequently called out to her mother at night when she awoke after scary dreams. She was severely afraid of thunderstorms and dogs. From an early age, she was also was very afraid of dying. In her teen and young-adult years, Jennise reported having some “social anxiety” where she described being really concerned about people’s perceptions of her and struggling to know what to say to peers at times. When she went off to college, she made an “active decision to put [her] difficulties behind [her] and become successful, wealthy, and independent.” She described channelling all of her energy into her schoolwork and careers goals. She did well in school and quickly became a lawyer. She met her spouse in law school and they got married shortly after graduating. When asked about the referral from her physician, Jennise reported that she recently visited her primary care physician again because she has been experiencing ongoing cold-like symptoms, including fatigue, body aches, and headaches. She has been having difficulty concentrating as well. When asked about this, she describes struggling to focus due to the fatigue and aches. Jennise has historically enjoyed recreational reading, but has given this up because, with her concentration difficulties, she cannot remember what she reads from one page to the next. She describes how last time she tried to read a novel, she had to re-read the same page 5 times. During the past year, Jennise’s symptoms have been evaluated by her primary care physician, an endocrinologist, a rheumatologist, and a neurologist. She has undergone multiple tests, but no specific etiology for her symptoms has been reached. Unsure of what else to recommend, doctors encouraged her to try to reduce her stress and engage in greater self-care to see if that helps. At her last visit with her primary care physician, he also recommended she see a psychologist and he made the referral.Jennise was tearful as she described that she quit her job as an attorney a few months ago because her symptoms prevented her from being effective at work. She reports that her average day now currently involves trying to manage her symptoms through rest, over the counter medications, and distraction. She also tries to get one or two household tasks completed each day. She reports that this last year has been “a blur” and she feels like she is “not sure where the time went or what [she has] been doing with [her] life.” When asked about the presence of other physical symptoms in the past, Jennise reported that she used to experience highly frequent and severe headaches. Jennise described how her spouse was caring and very supportive of her when she experienced these headaches. Her spouse would take time off of work to care for her. Jennise notes that her spouse and her have generally had very little time together due to their busy and intense careers. More recently, her spouse has stopped taking time off of work to be with her when she feels ill and seems to be demonstrating less patience with her.
