From 9:00 am -11:00 am each day, a group of 15 residents between the ages of 18 to 22 attend a training program to work on improving independent living skills, communication skills, and selecting leisure activities. You have been referred three separate cases to address the issue of rumination. Rumination includes voluntary and repetitive regurgitation of undigested or partially digested food. For all three cases, a medical evaluation and swallow study were completed and behavioral interventions were recommended by the treating physician. In addition, a functional analysis was completed by the BCBA. The hypothesized function of rumination, for each of the clients, is automatic reinforcement. You will evaluate the effects of non-contingent liquid presentation. Below is some information regarding each of the clients: Joshua: 22 y/o African American young adult who is dx with autism spectrum disorder and a severe cognitive delay. They engage in rumination for nearly 2 hours every day during the training program. They commonly ruminate after eating pudding, which is their preferred reinforcer. When ruminating, they first swallow food, then brings it up, gurgles the food, and swishes it in their mouth. Sometimes they spill out of their mouth. They are nonverbal and have limited communication skills. They will respond to their name about 60% of the time. They will orient to sound. They require hand-over-hand prompting for most tasks at the day program. They have problems with mobility and often moves very slowly. Joshua has lived in residential centers for the duration of their life. They have no contact with their family. They have been appointed a guardian ad litum. Jace: 18 y/o Caucasian student who is diagnosed with autism spectrum disorder and a moderate cognitive delay. Jace engages in rumination approximately one time per week. They seem to engage in this behavior when they are bored. Jace is nonverbal, but communicates pretty effectively using gestures and a few signs. Jace responds to their name nearly 100% of the time. Jace enjoys playing board games. They have strong imitation skills. They are ambulatory and is frequently on the go. Jace is compliant with task demands approximately 80% of the time. Rebecca: 19 y/o Caucasian young adult who is diagnosed with autism spectrum disorder and a moderate cognitive delay. They engage in rumination consistently throughout the 2 hr training program. Their preferred reinforcer is chocolate pudding, which they commonly ruminate. They also engage in high rates of self-injurious behavior (hitting her head with her hand), which has necessitated the use of a protective helmet. Rebecca is nonverbal and has been working to use a Picture Exchange Communication System (PECS). They are currently working on Phase 2 of PECS. They consistently respond to their name.
