Madge and Bob have been married for three years and are both 29 years old. They reported for marital counseling after their initially busy sex life took a disappointing turn. The marriage counselor decided to meet with them individually before meeting with them as a couple, as it became quickly apparent that both Madge and Bob had kept secrets from each other.

Madge reported she did not have orgasms during sexual intercourse with Bob, though she had been orgasmic in a previous relationship several years ago and could reach orgasm though masturbation, which she occasionally did. She reported that she sometimes faked orgasm with Bob because she didn’t want him to feel bad; however, she stated he was aware she was not having regular orgasms and that she thought he considered himself a failure. Madge stated she tried to help Bob feel more secure in his sexuality, but she reported that Bob was rarely able to sustain sexual intercourse more than a few seconds before he ejaculated, and this bothered both her and Bob a great deal. She felt her inability to reach orgasm was caused both by emotional problems, and by the fact that sexual intercourse ended so quickly She also reported she had recently noticed Bob seemed to “shut her out” during foreplay and sexual intercourse, not acknowledging anything she said or did and acting as though his mind was somewhere else.

Bob’s interview corroborated Madge’s story about his inability to delay ejaculation. He reported to the counselor that he had ordered special creams and studied research in an effort to find a way to reduce the stimulation he experienced during intercourse. He even tried to think about something else during intercourse with Madge in the hopes that thinking about baseball scores would keep his mind off of the sexual situation and allow him to continue intercourse longer. The counselor realized this might explain why Madge felt “shut out” during intercourse. Finally, Bob stated he was experiencing a lot of guilt because he had been masturbating 2-3 times a day and he was worried that Madge would find out. it was Bob’s belief that masturbating would allow him to withhold ejaculation longer during intercourse with Madge. He reported low self-esteem and serious concerns about the future of his marriage with Madge.
